Summary

Tirzepatide is a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist. In clinical research it has demonstrated significant reductions in body weight and glycemic markers, with effects exceeding those of GLP-1 mono-agonists in head-to-head comparisons. It mimics endogenous incretin hormones released after meals.

Think of it as a supercharged appetite suppressant. After you eat, your body releases hormones that tell you to stop eating and signal your pancreas to release insulin. Tirzepatide mimics two of those hormones at once — making it more effective than drugs that only mimic one. It's produced some of the largest weight losses ever recorded in a clinical drug trial.

Mechanism of Action

Tirzepatide binds and activates both GIP (GIPR) and GLP-1 (GLP-1R) receptors with near-equal potency. Activation of these receptors amplifies insulin secretion in a glucose-dependent manner, suppresses glucagon, slows gastric emptying, and reduces appetite via central and peripheral signaling pathways (hypothalamic arcuate nucleus). The dual agonism appears synergistic for adiposity reduction.

2.5 mg – 15 mg

Once weekly subcutaneous injection

Escalation protocol: 2.5 mg × 4 weeks, then 5 mg × 4 weeks, up to 15 mg. Research subjects are typically titrated to tolerate GI effects.

Reconstitution Guidance

BAC water: 3 mL

Resulting concentration: 3.3 mg/mL (for 10 mg vial)

Typical dose: 75 units on U-100 syringe = 2.5 mg

Add 3 mL sterile BAC water to 10 mg lyophilized vial. Mix by gentle rotation; do not shake.
