Summary
Tesamorelin is a stabilized analogue of GHRH with a trans-3-hexenoic acid modification that increases plasma half-life. The only GHRH analogue with FDA approval (for HIV-associated lipodystrophy), it has documented efficacy in reducing visceral adipose tissue in controlled trials.
Like sermorelin but engineered to last longer in the bloodstream. It's the only GHRH analogue the FDA has approved — specifically for reducing deep belly fat (visceral fat) in HIV patients on antiretroviral therapy. The GH it triggers causes the liver to produce IGF-1, which then targets fat around the organs for breakdown.
Mechanism of Action↓
GHRH receptor agonist with enhanced stability vs native GHRH. Stimulates pulsatile GH release from the pituitary. GH in turn stimulates IGF-1 production, which mediates lipolytic effects on visceral fat.
Half-life: ~26–38 minutes (significantly longer than sermorelin due to structural modification)
Research Dosing Ranges↓
1–2 mg per day
Once daily subcutaneous injection
FDA-approved dose for lipodystrophy: 2 mg/day. Research uses similar dosing.
Reconstitution Guidance↓
BAC water: 1 mL
Resulting concentration: mg/mL depends on vial size
Add 1 mL BAC water. Check vial size — common: 2 mg/1 mL = 2 mg/mL.
Storage & Stability↓
Refrigerate. After reconstitution use within 24 hours (FDA label) or up to 7 days (research practice).
Known Side Effects / Contraindications↓
Injection site reactions, fluid retention, arthralgia, carpal tunnel syndrome, glucose intolerance. Same class effects as GH.

Concentration: 5 mg ÷ 3 mL = 1.67 mg/mL (1667 µg/mL)
Dose needed: 250 µg = 0.2500 mg
Volume per dose: 0.2500 mg ÷ 1.67 mg/mL = 0.1500 mL
On U-100 syringe: 150.0 µL = 15.0 units
Total doses in vial: 5 mg ÷ 0.2500 mg = 20.0 doses
