Summary
SS-31 (elamipretide) is a cell-permeable tetrapeptide that targets cardiolipin on the inner mitochondrial membrane. It protects mitochondrial structure and function and is studied for heart failure, ischemia-reperfusion injury, and aging-associated mitochondrial dysfunction.
Mitochondria (your cellular power plants) have a critical structural lipid called cardiolipin that can get damaged during oxidative stress — causing the mitochondria to dysfunction and leak harmful signals. SS-31 goes directly into the mitochondria and protects cardiolipin from that damage, keeping the machinery working. It's studied for heart failure, kidney injury, and aging — conditions where mitochondrial dysfunction is a central problem.
Mechanism of Action↓
Selectively binds cardiolipin (a phospholipid of the inner mitochondrial membrane). Prevents cardiolipin from converting to a peroxidase during oxidative stress, protecting electron transport chain complex III/IV function. Reduces ROS generation and inhibits cytochrome c-mediated apoptosis.
Half-life: ~4 hours (subcutaneous); IV plasma clearance faster but tissue exposure sustained

Once daily subcutaneous injection
Most well-studied clinical dose is 40 mg/day SC (SPERICA trial, heart failure with preserved ejection fraction). Community protocols range 5–40 mg. For a 70 kg subject, 0.05–0.25 mg/kg/day = 3.5–17.5 mg/day.
