Summary

PT-141 (bremelanotide) is a cyclic heptapeptide melanocortin analogue derived from Melanotan II. It activates MC3R and MC4R centrally to produce sexual arousal in both men and women. An approved intranasal formulation (Vyleesi) is used for hypoactive sexual desire disorder (HSDD) in premenopausal women.

PT-141 works on the brain to create sexual desire — unlike Viagra or Cialis, which only work on blood vessels. This means it's effective in both men and women, and it produces psychological arousal, not just a physical response. A milder intranasal version (Vyleesi) is FDA-approved for women with low libido. The injectable version used in research is more potent. Nausea is the main side effect to manage — start low.

Mechanism of Action

MC4R agonism in the brain's limbic system (particularly the medial preoptic area) mediates sexual arousal and erectile/genital blood flow independently of the NO-cGMP pathway. This distinguishes it from PDE5 inhibitors and makes it active in both sexes.

Community consensus: 1–1.5 mg is the sweet spot for most users. Start at 0.5 mg to assess nausea response. Do not exceed 2 mg — stronger side effects without added benefit. Not recommended more than 2×/week.

Reconstitution Guidance

BAC water: 3 mL

Resulting concentration: 3.33 mg/mL (for 10 mg vial)

Typical dose: 30–45 units on U-100 syringe = 1–1.5 mg

Add 3 mL BAC water to 10 mg vial → 3.33 mg/mL. 1 mg dose = 300 µL = 30 units U-100.
