Summary

Mazdutide (IBI362) is an investigational GLP-1 and glucagon receptor dual agonist developed by Innovent Biologics (China). Phase 3 data showed up to ~15% body weight reduction. It is distinct from tirzepatide (GLP-1/GIP) — it pairs GLP-1 with glucagon, not GIP, adding an energy expenditure component via glucagon receptor activation.

Similar to tirzepatide but with a different 'second partner.' Tirzepatide pairs GLP-1 with GIP; mazdutide pairs GLP-1 with glucagon. The glucagon part is what makes it interesting — glucagon normally raises blood sugar, but in combination with GLP-1 the blood sugar effect is neutralized while glucagon's metabolic boost (increased calorie burning) is preserved. Mainly approved and studied in China.

Mechanism of Action

GLP-1 receptor agonism mediates satiety, slows gastric emptying, and provides incretin insulin secretion. Glucagon receptor (GCGR) co-agonism increases hepatic glucose output, thermogenesis, and lipolysis — complementary to GLP-1 with net metabolic benefit at moderate GCGR activation. The dual mechanism drives greater energy expenditure than GLP-1 mono-agonists.

Phase 3 titration: start at 2 mg/week × 4 weeks, escalate to 3, 4.5, up to 6 mg/week. Research doses up to 9–10 mg/week studied. Approved in China at 3–9 mg range. GI tolerability drives titration pace.
