Summary
Cagrilintide is a long-acting acylated amylin analogue developed by Novo Nordisk. Amylin is co-secreted with insulin from pancreatic beta cells and complements glucose regulation. In combination with semaglutide (CagriSema), it showed additive weight loss of ~15–22% body weight in phase 3 trials; an NDA was filed with the FDA in 2025.
Amylin is a hormone your pancreas releases alongside insulin after every meal. It slows digestion, suppresses glucagon (a hormone that raises blood sugar), and signals the brain that you're full. Cagrilintide mimics amylin but lasts a full week. Combined with semaglutide (sold as CagriSema), it produced weight loss approaching what's seen with surgical interventions. Novo Nordisk is seeking FDA approval.
Mechanism of Action↓
Amylin receptor agonist (calcitonin receptor + RAMP heterodimer). Slows gastric emptying, suppresses postprandial glucagon, and induces satiety via area postrema and hypothalamic signaling. The fatty acid acyl chain on cagrilintide provides albumin binding, extending half-life to ~7 days for once-weekly dosing.

Once weekly subcutaneous injection (titration schedule)
Standard titration: start at 0.25 mg/week × 4 weeks, then 0.5, 1.0, 1.7, up to 2.4 mg/week maintenance. GI tolerability drives titration pace. Phase 3 (REDEFINE) used 2.4 mg maintenance. Often paired with semaglutide 2.4 mg/week.
